If you authorise someone to pick up your diploma for you, please download and fill in this form. You need to fill the information about you and the person who will collect the diploma for you. After completing this form, please send this form together with the person's (who will pick up the diploma) passport picture to the CIP Administrative Office.
When your representative comes to CIP, he or she needs to show the passport. The office will check the passport at that time.
